SASH Coordinator (Brattleboro)

 

Brattleboro Housing Partnerships has an immediate opening for a part-time Support And Services at Home (SASH) Coordinator based in Brattleboro, Vermont. The primary function of the Support and Services at Home (SASH) Coordinator is to build trusting relationships with participants in SASH in order to develop a thorough knowledge of each SASH participant’s strengths and challenges as they pertain to remaining safely in his or her home.  The goal of the SASH model is to enhance the coordination of person-centered health and wellness services in order to assist residents to remain in their homes as their health and functional needs change.

Qualifications:  Must possess a Bachelors Degree in Social Work or equivalent combination of background and experience.  Previous experience coordinating services specifically for the senior or adult with disability client population is preferred.  Knowledge of area resources and programs available to seniors and adults with disabilities, including transportation, health services, and recreational activities is essential.  Demonstrated excellence in verbal and written communication is required. Must have knowledge of and an appreciation for the heritage, values, and wisdom of each resident and a commitment to the philosophy of a person’s choice to age at home.  Must possess a valid driver’s license.  A working knowledge of MSWord and Publisher is highly desirable.

Support and Services at Home (SASH) Coordinator

The Winooski Housing Authority is seeking a full-time SASH Coordinator.   The primary role of the Coordinator is to conduct outreach and provide referral and support to residents so that they are able to successfully age in place. The ideal candidate will have the ability to work effectively as part of a team of community providers and the ability to build trusting relationships with a diverse group of residents and community members.

Applicants must possess a Bachelor’s Degree in Human Services or a combination of background and experience working with the elderly or adults with disabilities. Outstanding organizational and communication skills are required.
