Our Impact

No one goes hungry on Molly Bennett’s watch. Molly is the SASH Coordinator based out of WWHT’s Windsor Village, where, for the last four years, she has helped connect residents and low-income seniors in the community with resources, services and supports that they need to continue to live as independently as possible. That includes bringing healthy food to all who need it.

Our Impact

This summer, SASH introduced on-site dental cleaning pilots across Vermont SASH panels, enhancing access to essential dental care. By addressing social determinants of health, SASH aimed to reduce barriers to dental care and improve oral health outcomes for participants, particularly those with chronic conditions such as high blood pressure and diabetes.

In the early morning hours of Tuesday, July 30, 2024, while people were asleep, the Lyndon/Saint Johnsbury areas were hit with a massive storm that dumped nearly 9.3 inches of rain on the already water-soaked communities. When a family in the Red Village community lost their home, SASH and RuralEdge staff stepped in to support them along with others who were impacted by this disaster.

Rutland Housing Authority (RHA), a public housing authority in Vermont, implemented Support and Services at Home (SASH), a program that provides health intervention to its elderly population to ensure they have a fair chance at living where they want. Since the program's inception, RHA has made a major difference for seniors in their community by supporting them in their own homes.

One of Downstreet’s SASH participants was being hospitalized for inpatient psychiatric treatment up to three times per month, but this year, a new chapter unfolded under their SASH Coordinator’s guidance. Together, they embarked on a mission to reduce hospital visits and foster a more stable, fulfilling life.

For over a decade, SASH has been improving health outcomes for older adults by connecting them to community-based supportive health and social services. Now, a new pilot program called SASH for All is bringing these services and resources to a broader population of Windham & Windsor Housing Trust (WWHT) residents.

Former Cathedral Square CEO Nancy Eldridge has been working with California Integrated Care at Home (CICH) to bring the SASH model to California. We recently had the opportunity to host representatives from various organizations in California who are planning to pilot SASH in Los Angeles County.

Cathedral Square and SASH staff recently had the opportunity to welcome several representatives from CMMI at Allard Square in South Burlington. State partners from the Agency of Human Services also participated and provided additional context about SASH from their perspective as a great model serving the state.

In the spring and summer of 2023, SASH teams around the state worked to address the opioid epidemic effecting our communities by learning about opioid addiction, sharing information with participants, and employing harm reduction strategies.
