Program Impact Snapshot

What stands out is the increase in group programs – nearly 800 groups were offered across SASH in the last two months! The ability to enter group programs in FAMCare is so important for showing what SASH does for participants. These groups foster social connections, promote physical wellness, provide emotional support, and so much more. Thank you for taking the time to document these groups, and for all the great work you do!
SASH Spotlight - Carol Keefe, Rutland Housing Authority
For the past 15 years, Carol has served as both a SASH Coordinator and Implementation Manager in Rutland, helping shape the growth and success of the SASH program in the region.
As Carol begins a new chapter as Resident Relations and Wellness Manager at the Rutland Housing Authority, she leaves behind an incredible legacy of collaboration, innovation, and dedication to helping older adults live healthier, more independent lives.

What Keeps Older Adults from Seeking Mental Health Care?
Pre-Panel Review Questionnaire
Beginning in July 2026, the SC & WN Pre-Panel Review JotForm will be updated to support more efficient and effective panel review meetings. The revised form will include additional quality measures, such as Up-to-Date and Due (U&D) Status, and assessment completion rate (%).
These updates will help teams identify documentation and assessment gaps before meetings, streamline preparation, and improve the consistency of quality reviews. By completing the JotForm in advance, staff can expect to reduce panel review meeting times from approximately 90 minutes to less than 60 minutes, saving about 30 minutes per review and allowing more time to focus on participant care, care coordination, and action planning.
Running reports is an important step in preparing a panel review. The Use and Disclosure Rates, SASH Coordinator Assessment Rates, and Wellness Nurse Assessment Rates reports, all under the QA Reports section, will give you overall rates at each of your panels. This lets you quickly get the information you need to fill out the pre-panel review JotForm and see how your panel is doing ahead of the review. Staff who need assistance accessing or interpreting this data are encouraged to contact the SASH Help Desk for support before implementation. The changes reflect our ongoing commitment to quality improvement and enhancing the panel review process for both staff and participants.
Healthy Aging Series
Casey Doerner completed the Professional in Age-Friendly Health Care certification course through the Institute for Healthcare Improvement. Part of that course was reviewing the changes that occur with aging. Check out the first segment on the peripheral nervous system.
