A recently published, peer-reviewed study has given us powerful evidence of what we have long known to be true: your work is measurably improving health, extending lives, and helping participants age with dignity and independence.

You all have made great progress documenting your work over the past two months. ED follow-up visits, blood pressure readings, and participants with 1-on-1 visits are all at the highest level since we started tracking in January. Keep up the good work!
SASH Spotlight - Autumn Dorman, Cornerstone Housing Partners
Autumn joined SASH a year ago and dove into 5 (FIVE) Panel's as our Wellness Nurse. She has done this with ease and grace. Her respectful and professional attitude with co-workers and participants have made her irreplaceable. Thank you Autumn!

It’s not a sprint, it’s a marathon. Actually, it’s a relay! In July Marci will be handing the WN Lead and Health System Educator baton back to Casey. Marci has dedicated the last 4 years to the SASH program and we are immensely thankful for her leadership. Read more here.

CHLP Submission Update
Thank you to the 74% of panels who have submitted their Community Healthy Living Plan (CHLP), we truly appreciate your time and effort. If you haven’t submitted yet, there are still a couple of months left, and support is available if needed.
Annual Reminder: Use and Disclosure Forms
Please remember to review your panel-specific Use and Disclosure (U&D) forms annually to ensure accuracy, including active MOUs and the correct Privacy Officer. While the updated VT U&D form (effective January 1, 2025) supports continued communication when renewals are difficult, U&Ds must still be updated yearly. Expired U&Ds will not receive Bamboo Health pings.
