SASH Receives 2024 IPRO Quality Award for Chronic Disease Management

SOUTH BURLINGTON, VT – SASH® (Support and Services at Home) has been named a recipient of a 2024 IPRO Quality Award presented in recognition of its significant achievements in improving healthcare quality. SASH is a participant in the IPRO Quality Innovation Network-Quality Improvement Organization (QIN-QIO). QIN-QIO is a federal program focused on improving the quality of care provided to Medicare patients across the United States. It is funded by the Centers for Medicare & Medicaid (CMS).
SASH was recognized for chronic disease management. SASH worked with Healthcentric Advisors, a collaborator in the IPRO QIN-QIO, to achieve measurable quality improvement outcomes working with the IPRO QIN-QIO on a project that provided a focused intervention to improve hypertension among older adults.
“We are honored to receive this award and appreciate the recognition of all the amazing work SASH wellness nurses and coordinators do to help SASH participants better manage their blood pressure,” shared SASH Assistant Director Casey Doerner. “SASH staff have had a profound impact throughout Vermont by loaning blood pressure cuffs, helping meet social needs, providing medication reconciliation, exercise programing, and education, and collaborating with care teams on participants’ health.”
“Recognizing providers that achieve measurable improvement in their quality outcomes is an important aspect of the work we do,” said IPRO CEO Ted Will. “It allows us to showcase the meaningful progress that is made through our partnership with provider organizations that implement evidence-based interventions into their processes.”
