Become a SASH Participant

SASH is available to Vermonters on Medicare free of charge. We provide a helping hand in staying safe, healthy and living at home. We work with you to assess your health and wellness needs, connect you with resources, and help you meet your goals — goals you decide for yourself.

If you or someone you know could benefit from SASH, select the appropriate region, fill out the form, and we’ll be in touch. You also may print the form, complete it by hand, and fax or email it back to us. We'll take it from there!
